Vaccination Information

For the 2017-2018 school year the Ohio Department of Health (ODH), under Ohio Revised Code 3701.13 and 3313.671, requires all students entering the 7th Grade to provide written proof that they have received a dose of Tdap (tetanus, diphtheria and pertussis) and Meningococcal (meningitis) vaccines. Students entering 12th Grade must provide written proof that they have received the Meningococcal (meningitis) vaccine after the age of 16. Please see the Clinic Resources tab under Parents on our District Website or by following this link. 

 

You may obtain the Tdap and/or Meningococcal Vaccines from your private physician or the Galion City Health Department. You can schedule an appointment with the Galion City Health Department by calling (419) 468-1075.
