2024-25 Information
For the 2024-25 school year, the Ohio Department of Health (ODH), under Ohio Revised Code 3701.13 and 3313.671, requires all students entering the 7th grade to provide written proof that they have received a dose of Tap (tetanus, diphtheria and acellular pertussis) and Meningococcal (MCV4) vaccines.
Also, all students entering the 12th grade must provide written proof that they have received the Meningococcal (MCV4} vaccine after the age of 16. Please note that fi your 12th grade student had a dose of Meningitis vaccine PRIOR to age 16 they are required to have a second dose.
Your student will need written proof of vaccine/or an appointment scheduled by August 31, 2023; or they will be excluded from attending school beginning September 5, 2024. Immunization exemption must be submitted with appropriate documentation prior to September 5, 2024.
You may choose either your primary health care provider or your local health department to receive the vaccine(s). An immunization clinic schedule for Crawford County Public Health and Galion Health Department is included below. Please have your health insurance information when you call for the appointment.
