NASN Treatment Information

As required by the Ohio Department of Health, students in the following grades will have these screenings done:

• Kindergarten: Vision & Hearing (by 11/11 of the school year)

• First Grade: Vision & Hearing (by 11/1 of the school year)

• Third Grade: Vision & Hearing

• Fifth Grade: Vision & Hearing

• Seventh Grade: Vision ONLY

• Ninth Grade: Vision & Hearing

• Eleventh Grade: Vision & Hearing (students going to Pioneer will be screened at Pioneer and results communicated back to Galion High School for the health record)

Please note: IF students pass the screens, NO notification will be sent home. Results will be recorded on the health records of the student's file. If students do not pass the first screen, they will be re-screened in a six-week time period. If they do not pass the second screen, a referral will be mailed home to the parent or guardian. Please have your student seen by the appropriate health care professional and let the school know the results of the visit by returning the referral form as directed.
